Standard #: 8VAC20-780-130-A Description: 8VAC20-780-130. A.
Based on record review and interview, it was determined that the center did not ensure to that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #1 and Child #2 (date of enrollment are both 9/05/2023) do not have their immunization documentation required prior to attendance in their records.
2. Staff #2 confirmed that Child #1and Child #2 do not have their immunization records in their case file.Plan of Correction: The center responded with the following: Staff #1 will speak with Child #1 and Child #2?s parents to obtain the child?s immunization record and place it in their file.
Standard #: 8VAC20-780-140-A Description: Based on record review and interview, it was determined that the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 and Child #2 (date of enrollment for both are 9/5/2023) do not have physical examination documentation in their record.
2. Staff #1 confirmed that Child #1 and Child #2 do not have physical examination documents in their record.Plan of Correction: The center responded with the following: Staff #1 will speak with Child #1 and Child #2?s parents to obtain the child?s physical. The center will audit the files and request from the parents any missing physicals to the record.
