Technical Assistance:
Discussed the medication should be in the original box or prescription label attached, attendance sheet requirements, allergy list requirements and hand washing.

Comments:
An unannounced renewal inspection was conducted on November 16, 2023, from 9:00am-11:00am. At the time of entrance, 19 children were in care with 9 staff members present. The inspector reviewed 2 children?s records and 2 staff records. Children were observed playing with toys, diaper changes, washing hands and finishing snack. Infants were being fed a bottle and being held, swinging in an infant swing and sitting in chairs. The physical space, evacuation drills and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of staff records, the center did not obtain the central registry finding within 30 days of employment.
Evidence:
1. Staff #1 (Date of hire 8/31/2023) had the central registry findings in the file, but it was dated 10/31/2023. The results came in after 30 days of employment due to being sent back for corrections.

Plan of Correction: Central Registry contacted the employee to correct
something. She did but we did not receive the results
within 30 days.
Director had to send a new application and received
that one in days.
We now know that we have to followup with Central
Registry within 30days.

Standard #: 8VAC20-780-280-B
Description: Based on observation, Hazardous substances such as cleaning materials were not kept in a
locked place using a safe locking method that prevents access by children.
Evidence:
1. In Room 109, a container of disinfectant wipes, Febreze, Bleach water and disinfectant spray was inside a cabinet above the sink and the child lock was not latched.

Plan of Correction: Staff member and all the other staff have been
reminded to lock cabinets after cleaning the diaper
changing areas.

Standard #: 8VAC20-780-290-A-3
Description: Repeat Violation
Based on observation, the center failed to cover all unused electrical outlets accessible to
children under five years of age with child resistant protective covers.
Evidence:
1. In Room 100 and Room 101, an unused outlet was covered with tape in each classroom and did not have child resistant protective covers on it.

Plan of Correction: We have covered both outlets with covers and
moved the charts out of the way.
