A subsidy health and safety inspection was conducted today. The vendor was present with 4 children in care and one of her own children, a total of 8 points. Observed the children playing games, playing with toys, doing puzzles and watching a movie. The space used for child care is full of different interest areas and supplies for the children to explore. The vendor will transport the children in care and will provide meals. The children had fruit with a dipping sauce for snack. This inspection was started at 11:35AM and ended at 12:30 PM . Please call me at 703-203-8659 if you have any questions regarding this visit.

Standard #: VENDHOM-000-006-B
Description: Based on the review of the vendors written information for children, some information is missing. Evidence: The vendor is missing emergency contact addresses for all the 4 children's records reviewed today. Child a,b,c and d needs emergency contact addresses.

Plan of Correction: I will ask the parents for the emergency contact addresses.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
