Based on observation, facility did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The following was observed during inspection:
1. One of three toilets in the hall bathroom is not operable with a sign "out of order" on the lid.

3. Two tricycles on the playground were missing a foot pedal.
4. The privacy fence on one portion of the playground did not reach the ground level (approximately 1' opening) and could allow a child to crawl out of the play area. Owner stated trees had recently been removed that had blocked the openings.Plan of Correction: A plumber has been contacted to come repair the leak from the toilet. The plumber must do repair when the weather is appropriate.
The signs and tricycles will be removed from the playground today.

Resilient surfacing will be raked to appropriate areas when weather allows. Until that is done, equipment will not be used children.
