Technical Assistance:
1. COVID-19 health and safety guidance reviewed with the vendor.
2. The provider's First Aid/CPR expired in April 2020, and has been unable to renew due to the temporary suspension of classes. The vendor reported that she had made contact with an instructor, who will contact her when classes resume. VDSS is exercising temporary leniency on this requirement due to the health pandemic.
3. Forms provided to the vendor: child enrollment, attendance, emergency drill, sick child policy, emergency medical authorization, and emergency preparedness plan.

Comments:
This inspection was conducted by the subsidy inspector using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time.

An initial scheduled subsidy health and safety inspection (SHSI) was conducted today to determine compliance with subsidy vendor health and safety requirements. There were no children during the inspection, though 5 are reported to be enrolled. The inspector emailed the vendor a list of items required to complete the inspection. The inspector reviewed 5 child records, the vendor's record, and 1 household member record submitted by the vendor to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or laws, and violations were documented on the violation notice issued.The vendor will have 10-days to correct non-compliances and submit documentation of such to the inspector, as indicated on the notice. Violations:
Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 5 of 5 children's records did not include required information.

Evidence:

1. The records for Child #1, #2, #3, #4, and #5 did not include: the child's home address; name(s) of each parent and their home/work addresses and phone numbers; address for the emergency contact person; names of individuals authorized to pick up the child; and a signed written statement that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon a possible, as needed.

2. The vendor told the inspector that she did not have all the information, and had asked the parent to provide it by 7/8/20.

Plan of Correction: The vendor will have an individual enrollment record with required information completed for each child.

Standard #: 22VAC40-665-400-A
Description: Based on records requested and submitted by the vendor, it was determined that a written emergency preparedness plan, that includes caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. The plan shall address the most likely to occur emergency scenario or scenarios, including but not limited to fire, severe storms, flooding, tornadoes, loss of utilities, earthquakes, intruders, violence on or near the premises, chemical spills, and facility damage or other situations that may require evacuation, lockdown or shelter-in-place, has not been completed. Evidence: The vendor did not submit the requested written emergency preparedness plan for review.

Plan of Correction: The vendor will complete an emergency plan prior to subsidy approval. A copy will be sent to the inspector for review.

Standard #: 22VAC40-665-410-B
Description: Based on records requested and submitted by the vendor, it was determined that a record of emergency drills practiced for the prior 1 year have not been maintained. Evidence: The vendor did not submit the requested drill records for review.

Plan of Correction: Fire drills, lock down drills, and shelter-in-place drills will be practiced and recorded as required. The records will be kept for a minimum of 12 months.
