An unannounced renewal inspection was conducted today, February 16, 2024, to determine the center?s compliance with licensing standards. The licensing inspector was on site from approximately 11:00 am until 3:00 pm. The Director was present during the inspection. There were 46 children present, ranging in ages from four months to five years, with seven staff supervising. An additional staff arrived shortly after the Inspector?s arrival. A group of children were observed outside playing on the playground with small trikes and in the playhouse. Later children worked puzzles, sorted counters, played with the iPad and toys. Lunch time and nap time were also observed.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and ten staff records were reviewed. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Repeat/Repeat Violation
Based on review of ten staff records, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children for one staff member.
Evidence:

Standard #: 8VAC20-780-160-C Description: Based on review of ten staff records, the center did not obtain the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening or testing for two staff members.

Standard #: 8VAC20-780-40-M Description: Based on interview and review of documentation, the center did not maintain in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence:
1. The center?s allergies, sensitivities, and dietary restriction list was not dated.
2. Laminated copies of the allergy, sensitivities, and dietary restriction list were observed sitting on top of a table at the entrance of the center, outside the door to the office. The list was not kept confidential nor were they in each room or area where children were in care.
3. Staff #7 stated she had one child with a milk sensitivity however she did not have a copy of the allergy, sensitivities, and dietary restriction list in the classroom. Based on review of ten staff records, the center did not obtain all required information
for one staff member?s record.
Evidence:
1. Staff #3 (employment 11/16/2022) was identified as a program leader during the
inspection.
2. The records for Staff #3 did not contain documentation of education required for the job
position.

Standard #: 8VAC20-780-350-B-2 Description: Based on observation and interview, the center did not maintain the required staff to children
ratio for children 16 months to 24 months of age.
Evidence:
1. The required staff-to-children ratio for children 16 months to 24 months of age is one staff for every five children. At approximately 11:15 am on February 16, 2024, seven children ages 18 months through two years were observed in care with staff #7 supervising. One additional staff was required.

Based on observation and interview, the center did not ensure the required staff-to-child ratios
were maintained.
Evidence:
1. Eight children (four months to three years of age) were being supervised by Staff #8. Staff #8 was the only staff member present in the classroom. The required staff-to-children ratio is applied to the youngest child in the group and is applicable to the entire group. The required staff-to-children ratio was one staff member for every four children present.
2. The director arrived at approximately 11:20 am and stepped into the infant room as the second staff.

Standard #: 8VAC20-780-510-P Description: Based on review of medications and authorizations, the center did not ensure when a medication authorization expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up within 14 days will be disposed of by the center.
Evidence:

Standard #: 8VAC20-780-570-A Description: Based on observation, the center did not ensure when a child is placed in an infant seat or highchair, the protective belt is fastened securely.
Evidence:
1. As the Licensing inspector was entering the infant room, child #2, twelve months in age, was observed standing in the highchair with one foot in the seat and one foot on the highchair tray.
2. The Inspector called out to staff #8, who ran across the room and grabbed the child.
3. The protective belt had been fastened however it was not secure. The belt had a knot that prevented the belt from fitting snuggly.Plan of Correction: Not available online. Contact Inspector for more information.
