Comments:
An unannounced monitoring inspection was conducted with the director on February 26, 2024, beginning at 10:30 a.m. and ending at 12:40 p.m. There were 20 children, ages 10 months through 5 years old, in the direct care of 7 staff. The children were observed playing with age appropriate toys, coloring, and playing outside on the playground. Five children?s records were reviewed and in compliance. Three staff records were reviewed. There was one medication reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 1/30/2024. The most recent shelter-in-place drill was documented as 9/27/2023. The most recent lockdown drill was documented as 12/27/2023. The most recent health inspection was dated 3/17/2023. The most recent fire inspection was dated 12/1/2023.

Description: Based on a review of documentation, the facility failed to obtain a central registry finding for staff within 30 days of employment or volunteer service. Evidence: Staff A (hire:7/10/23) and staff B (hire:1/3/2024) did not have documentation of a central registry finding in their records.

Description: REPEAT VIOLATION - Based on observation and staff interview, the facility failed to ensure that the resilient surfacing complied with the minimum safety standards where playground equipment is provided. Evidence: The resilient surfacing was less than one inch in the fall zones around the playground equipment. Raw dirt was visible in some areas of the fall zones. The minimum requirement for resilient surfacing in the fall zones is 6 inches of playground mulch.

Description: Based on a review of documentation and staff interview, it was determined the center failed to have atleast one staff member who meets the qualifications of a program leader or program director shall be regularly present for each grouping of children. Evidence: The teacher in the infant room does not possess program leader qualifications. The teacher in the toddler classroom does not possess program leader qualifications. The teacher in the early preK classroom does not possess program leader qualifications. The two teachers in the preK jr classroom do not possess program leader qualifications.

Description: Based on observation and staff interview, the center filed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: There were three children, 2 years and under, alone in a classroom. The staff was observed in the adjoining room talking to two other staff members.

Description: Based on observation, the center failed to ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: In the infant room, there was one 10 month old child seated in the feeding table that was not secured with a protective belt. In the toddler room, there was one 15 month old child that was seated in the feeding table and not secured with a protective belt.
