2. Form provided: daily attendance.

Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time.

An annual subsidy health and safety inspection was conducted on 7/13/20. The vendor was contacted by phone to initiate the inspection. The vendor reported that no children have been in her care since March 2020 due to the health pandemic. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed the vendor and 1 household member record, as the vendor reported she has not maintained children's records. Information gathered during the inspection determined non-compliances with applicable standards or laws, and violations were documented on the violation notice issued to the provider. Description: Based on records reviewed, it was determined that a written hard copy record of daily attendance that documents the arrival + departure of each child in care as it occurs has not been maintained by the vendor. Evidence: Attendance records provided were printed from the subsidy payment/swipe machine. The vendor said that she shredded the subsidy records.

Standard #: 22VAC40-665-160-D
Description: Based on interview conducted, it was determined that records, reports, and information required has not been kept and maintained in the home for 5 years after termination of services. Evidence: When the inspector requested the enrollment records for Child #1 and #2, the vendor stated that she no longer had the records, and that she had shredded the records since the children have not been in attendance for a period of time.
