Comments:
An unannounced renewal inspection was initiated on February 12, 2022, and concluded on February 22, 2024. There were 122 children in care ages four months through five years old. There were 17 staff, and three administrative staff present during the inspection. A walking tour of the center inside and outside play space was completed. Children and staff records, annual inspections, required postings, first aid kits on each floor, emergency equipment, emergency evacuation drills and board member records were reviewed. Violations regarding resilient surface, unlocked chemicals, hand washing, sanitation, diaper changing, and evacuation drills were observed and discussed with administrators during the exit meeting at the conclusion of the onsite inspection.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that areas are maintained in safe and operable condition.
Evidence:
There are trip hazards throughout of the outside play area due the poured-in-place resilient surface separating in many areas. The separation are wide enough in some areas to allow for a child?s foot to become wedged in the gap which may lead to a child tripping.

Plan of Correction: Playground Trip hazard - Owners met with church leadership on Monday, 3/27/24 to address the playground concerns and to come up with a plan to correct them. A nationwide playground equipment and installation company was contacted for a quote and this was reviewed during that meeting. The next steps include meeting with the Church?s Board of Directors for approval and determining how soon these funds can be obtained in partnership between Baylake Academy and Baylake United Methodist Church as the playground remodel is estimated to cost over $100,000. It is our goal to have this completed by no later than December 2024. If approved, the playground remodel is expected to take 2-3 weeks to complete. During this time, remodeled areas (footprint of the poured rubber surface) will be roped off and the two astroturf fields will be utilized for outside time.

Description: Based on observation the center failed to ensure that hazardous substances are kept in a locked place.
Evidence:
In the infant room there was a spray bottle on top of the paper towel dispenser, an unlocked place.

Plan of Correction: Unlocked chemicals - in the training held on the 19th we discussed the importance of locking up all soap/water and bleach/water solutions - we have also purchased more locks for cabinets to ensure they all had locks with the solutions. We have done routine checks to ensure teachers are continuing to lock up chemicals not in use.

Description: Based on observation the center failed to ensure that staff wash their hands with soap and running after contact with bodily fluids.
Evidence:
In the toddler room the staff wiped the runny noses two children in care without washing her hands between each nose wipe.

Plan of Correction: Hand Washing - In the training held on the 19th we discussed the importance of hand washing after diapering, eating, and coming in contact with any bodily fluids. We have also created a handwashing schedule for the younger classrooms to implement while transitioning. We have done routine checks to ensure teachers are following this schedule.

Description: Based on observation the center failed to ensure that a nonabsorbent surface is used for diapering or changing younger than three years.

Evidence:
In the two-year-old room, Child #1, a two-year-old, was changed while standing up in front of the toilet rather than on a diaper changing nonabsorbent surface.

Plan of Correction: Diapering in front of toilet - Went over in training on the 19th that all diaper changes regardless of age need to be on the diaper changing table provided. We have done routine checks to ensure teachers are continuing to change students in the diapering areas provided.

Description: Based on documentation review the center failed to ensure that emergency evacuation drills are practiced monthly.
Evidence:
There was not documentation that an emergency evacuation drill was held during the month of January 2024.

Plan of Correction: Fire Drill - The fire drill was completed on February 9th, 2024; however, it was not documented on the physical sheet, it was digitally documented in the school calendar. We have and will continue to document all drills on the physical sheet moving forward.

Standard #: 8VAC20-780-550-E
Description: Based on documentation review the center failed to ensure that shelter in place procedures are practiced a minimum of twice per year.
Evidence:
Documentation provided during the inspection shows two shelter-in-place drills were completed from July 5, 2022, through January 5, 2025, where five would be required.

Plan of Correction: Completed a shelter-in-place drill on March 13th, 2024. We will continue to complete the sheet once performed.
