An unannounced complaint investigation was initiated on 1/31/2024 and concluded on 3/28/2024 in response to a serious injury report received on 1/31/2024 and a complaint received by the licensing office on 02/15/2024 regarding supervision. Interviews were conducted relating to the allegations on 02/06/2024, 02/20/2024, and 03/04/2024. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to procedures for emergencies are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-550-P Complaint related: Yes Description: Based on record review and interview it was determined the center did not maintain a written record of children?s serious and minor injuries.
Evidence:
1. There was no injury report in the file for the injury on 2/14/24 for child #2.
2. Staff confirmed that they have not been documenting children?s injuries as required by 8VAC20-780-550.Plan of Correction: Our system has a written record of the injury. The inspector suggested we use a paper system that is more thorough than our software application. We have contacted our systems manager to request a change to the software. We will use paper until the change occurs.
