Comments:
An announced initial inspection was conducted between 11:30 A.M. and 12:45 P.M. on April 1, 2024. The licensing inspector conducted a tour of the physical plant, verified floor and site plans, and reviewed the background check findings for the applicant/agents and the program director.
As no children were in care at the time of the inspection, the center representative was reminded that when one or more children are in care there shall always be one staff member who meets the qualifications of a program leader or program director and an immediately available staff
member, volunteer or other employee who is at least 16 years of age with direct means for communication between the two of them. Policies and procedures submitted with the initial application were reviewed prior to the inspection. Areas of non-compliance are identified in the
Violation Notice.

Please return a signed and dated copy of each page of this notice to the licensing inspector no later than 5:00pm on April 8, 2024. If you?d like to include a plan or correction and date to be corrected for each violation cited, please include that information with the signed and dated copy of the Violation Notice by the deadline. Plans of correction should include steps to correct the noncompliance with the standard and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289-036-B-4
Description: Based on record review, the center failed to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Applicant #4 indicated on a sworn disclosure statement dated 02/28/2024 that they had lived outside of Virginia within the last five years.
2. There was not a completed out of state central registry finding in the record for Applicant #4.

Description: Based on record review, the center failed to have required background checks at the time of initial application for licensure.

Evidence:

Description: Based on document review, the center failed to ensure the emergency preparedness plan contained all required components.

Evidence:
The emergency preparedness plan provided by the center was missing the following procedural components: establishment of center emergency officer and back-up officer to include 24-hour contact telephone number, means of emergency notification of the health department, continuity of operations to ensure that essential functions are maintained during an emergency, staff training requirements, plan review and updates, primary and secondary means of egress for evacuation and shelter-in-place, securing of essential documents (including parent contact information and information on allergies of food intolerances) for evacuation and shelter-in-place and methods to ensure any health care needs (including medication and care plans, emergency contact information for staff) are taken to assembly point for evacuation and shelter-in-place.
