A self-report inspection was initiated on March 26, 2024. The self report was in regards staffing and supervision. Video footage was reviewed, documentation and staff was interviewed. The inspector reviewed compliance areas of staffing and supervision, programs and administrative.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-E Description: Based on interview and review, the center did not ensure that the center's activities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
1. Child Supervision Guidelines states: "Children must be supervised, in the direct line of sight and within ear shot of a staff member, at all times. All state mandated adult to child ratios must be followed at all times. "
2. Center policy also states: "Face-to-Name means staff must see the child's face and say the child's name when confirming all children are accounted for throughout the day. Face to names must be used to document staff to child ratios throughout the day: hourly, children arriving or leaving the classroom, transitioning children from one location to another (playground, fieldtrips, electives etc), combining and or blending classrooms. Face to Name sheets is kept current at all times."
3. The Face to Name sheet for the toddler room did not have Child #1 checked in the classroom. The face to name did not have child #1 checked out of the 2's classroom. Staff #1 and Staff #2 confirmed they did not check child #1 in and out of the classrooms when child #1 transitioned. Staff confirmed name to face sheet was not current.
Child #1 was not supervised, in the direct line of sight within ear shot of a staff member for approximately nine minutes.Plan of Correction: -Staff will turn in lesson plans to management as well as prepare all materials for the following week every Friday to ensure all materials are ready for instruction.
-Staff will effectively position themselves to always be able to see children.
-Staff will anticipate challenging behaviors and establish a plan to teach a positive skill.
-Center leadership conducts weekly classroom observation to ensure all supervision policies and
procedures are being followed according to company and licensing standards.
Staff were coached on effective positioning and anticipating challenging behaviors beginning
Tuesday, March 19, 2024. Weekly classroom observations began Monday, March 25, 2024.
Standard #: 8VAC20-780-340-A Description: Based on interviews and video footage, the licensee did not ensure staff are supervising children, they should always ensure their care, protection, and guidance.
Evidence:
On March 18, 2024, a two-year-old child was left alone in a classroom for approximately nine minutes without any staff supervision.Plan of Correction: -Providing weekly 1:1 training and coaching to ensure enhanced understanding on proper transitioning procedures
-Providing weekly classroom coaching to establish effective communication and teacher positioning to ensure all children are in sight and sound.
-Staff will clock in and out during all transitions to accurately maintain proper ratio and
supervision guidelines.
Initial training and coaching were implemented during our staff meeting on Wednesday, March 27th.
Coaching is being conducted on a weekly basis to ensure that communication and teacher positioning is being continued for effective child supervision.
Standard #: 8VAC20-780-340-F Description: Based on interviews and video footage, the licensee did not ensure that children under 10 years of age are in sight and sound supervision of staff.
Evidence:
1. On March 18, 2024, child #1 ( 2 years old) was left alone in the toddler classroom without sight and sound supervision for approximately 9 minutes. Staff #1and Staff #2 were responsible for the child's care.
2. After viewing video footage, it was determined that Child #1 was in the toddler classroom alone for 9 minutes freely playing. Staff #1 went out to playground with the toddler's. Staff #3 went into the 2 year old classroom to start her shift.
3. Staff #4 confirmed she looked into the window from the hallway and saw child #1 alone in classroom playing and notified staff.Plan of Correction: -Staff will not transition children without notifying management.
-Staff trained on proper transitioning procedures to effectively manage transitioning children:
? Face to name transition sheets
? Active supervision procedures
? Child safety procedures
All staff were trained on these policies and procedures Wednesday, March 27th during our staff meeting.
