Standard #: 22.1-289.035-B-4 Description: Based on record review, the center failed to obtain the results of a criminal fingerprint background check from any state in which the individual has resided in the preceding five years prior to the first day of employment.
Evidence:
1. The record for staff #2, date of employment 05/25/2023, indicated that staff #2 lived in another state outside of Virginia in the preceding 5 years prior to employment. The criminal fingerprint search for that state was received on 02/20/2024. The results were due on or before 05/25/2023.
2. The record for staff #3, date of employment 12/13/2022, indicated that staff #3 lived in another state outside of Virginia in the preceding 5 years prior to employment. The criminal fingerprint search for that state was received on 01/25/2024. The results were due on or before 12/13/2024.Plan of Correction: Administration will work with HR to ensure criminal backgrounds are done in the allotted time frame.
