Evidence:
1. Of the injury reports were reviewed five did not contain the required future action to prevent recurrence.
2. Staff confirmed that these five injury reports did not contain future action to prevent recurrence.Plan of Correction: Center will correct injury reports to include a place for future action to prevent recurrence.
