An unannounced investigation was initiated on 4/17/24 in response to a self reported incident being received in our office on 4/12/24.
The investigation was conducted jointly with another local agency.
The incident occurred on 04/12/24 and involved a three year old child being unsupervised. Observations were made, documentation was reviewed, and interviews were completed with staff. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please be aware that the plan of correction must be provided within 5 business days of receipt of the inspection notes if you want it displayed on the website.

Standard #: 8VAC20-780-40-E Description: Based on multiple staff statements, the center failed to follow the policy for knowing where children are at all times.
Evidence:
On 4/12/24:
1. Staff A and B stated no name to face check was conducted prior to leaving their classroom.
2. Staff A and B stated no name to face check was conducted prior to leaving the Gross Motor Room (GMR).
3. Staff A stated no bathroom check was conducted prior to leaving the GMR.
4. Staff did not conduct a 30 minute head count tracker while in the GMR. Staff thought there were 18 children present, when in actuality there were 19 children.
5. Staff C stated the 30 minute tracking sheet was not completed for the Children's House (CH) 4 room on 4/12/24.Plan of Correction: Staff complete the West Alex Roll Call sheet daily which includes:
* 30-minute head count tracker
? Face-to Name tracker
? Guide sign-in and out
In addition, all children are signed in to the center by their parent/pick-up person and by the guide who receives them.
Standard #: 8VAC20-780-340-A Description: Based on staff interviews, the center failed to ensure care, protection and guidance for a child in care.
Evidence:
1.On 4/12/24 Staff A and B were unaware that they left Child #1 (three years old) unattended in the Gross Motor Room (GMR) at approximately 12:00PM. The GMR is on the first floor of the facility, while the rest of the facility is on the third floor of the building. (Child 1)
2. Child 1 was found at approximately 12:30PM by a staff member bringing another group of children to the GMR. The child was crying and saying that they "wanted their mom".Plan of Correction: Staff complete the West Alex Roll Call sheet daily which includes:

? Face-to Name tracker
? Guide sign-in and out
In addition, all children are signed in to the center by their parent/pick-up person and by the guide who receives them. When leaving the GMR, staff check bathrooms and walk the room before exiting.
Standard #: 8VAC20-780-340-F Description: Based on staff statements, a child under 10 years of age was not kept within actual sight and sound supervision.
Evidence:
1. On 4/12/24 a three year old child was not within sight and sound supervision of staff when their classroom left the GMR on the first floor of the building at approximately 12:00PM leaving the alone in the room. (Child1)
2. The child was found alone in the room at approximately 12:30 by a staff member bringing another group of children to the room.Plan of Correction: Staff complete the West Alex Roll Call sheet daily which includes:
