An unannounced monitoring inspection was conducted on 4/26/2024 from 10:00am to 12:53pm. There were 192 children (2-5yrs) directly supervised by 40 staff. The physical plant, 9 staff records, 10 children?s records, 7 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for and eating lunch. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-60-A Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: reviewed 10 children's records and found that:
1) Child 5 did not have documentation of authorized pick-ups
2) Child 3 did not have documentation of addresses for either emergency contact
3) Child 5 did not have documentation of emergency contacts
4) Child 2 had documented a parent as an emergency contactPlan of Correction: The Center is located in a highly transient area and serves many military and Dept. of State families that relocate often. Many of these families do not have additional emergency contacts other than themselves. Some don?t have anyone else in the area that they know well enough to authorize them to pick up their children. Others don?t feel safe having other people pick up their children. Additional Authorized Pick-up people:
The Center will ask parents to write down N/A in the corresponding form when a list
of authorized pick-up people cannot be provided.
Emergency Contacts: The Center will explain to parents who do not wish to provide emergency contacts other than themselves, that per VA Licensing requirements, their child cannot attend school until they provide at least two emergency contacts different from themselves.
Standard #: 8VAC20-780-280-B Description: Based on observation, the facility did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed a spray bottle with soap water was accessible to children on sink counterPlan of Correction: Teachers were reminded that soapy water solutions need to be stored out of children's
reach after each use and to remind volunteers of this rule. A ?return to storage after use? label will be added to the cleaning solutions, and a ?Store & Lock cleaning solutions after use? will be posted on the designated storage space.
Standard #: 8VAC20-780-520-B Description: Based on observation, the facility did not ensure that sunscreen shall be inaccessible to children under 5 years of age.
Evidence: observed sunscreen accessible to children in an unlocked cabinet under the diaper changing table in room 157.Plan of Correction: The Center reminded teachers that the diaper changing station childproof lock must be engaged at all times. A sign with a reminder will be posted on the doors of the diaper changing stations when applicable.
Standard #: 8VAC20-780-560-G Description: Based on observation, the facility did not ensure that when food is brough from home, the following shall apply: the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
Evidence: observed in room 152 a lunch box that was not labeled with a name or a date.Plan of Correction: The Center reminded parents and teachers of the importance of labeling lunchboxes with the date and name of the child.
