An investigation was initiated in conjunction with a monitoring inspection from 8:15 a.m. - 10:15 a.m. in response to a complaint received by the department regarding supervision. interviews were conducted, documentation, child records, and staff records were reviewed. The complaint regarding supervision is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-350-B-1 Complaint related: Yes Description: Based on interviews and review of documentation it was determined that a one to 4 staff/child ratio was not always maintained. Evidence: Upon interviews and review of the attendance record it was determined that Staff A was caring for five infants beginning at 7:13 a.m. the sixth child arrived at 7:23 a.m. Two children were then moved to the toddler room at 7:38 a.m. decreasing the number of children back to four. Three additional children arrived at 7:45, 7:55, and 7:55 a.m. increasing the number of children with staff A to seven. At 8:05 a.m. staff B arrived to the classroom. Plan of Correction: School will initiate a system to communicate to parents when we can no longer accommodate children in certain classrooms to maintain ratios.
Standard #: 8VAC20-780-350-B-3 Complaint related: Yes Description: Based on interviews and observation it was determined that a one to eight staff/child ratio was not always maintained. Evidence: The LI (licensing inspector) observed the two year old classroom to have 17 children in care being supervised by two staff. At 9:00 a.m. four children were moved to the Pre-School 1 classroom. Plan of Correction: School will initiate a system to communicate to parents when we can no longer accommodate children in certain classrooms to maintain ratios.
Standard #: 8VAC20-780-350-Q Complaint related: Yes Description: Based on interviews and documentation it was determined that children are repeatedly reassigned from their regular group in effort to maintain the required ratio. Evidence: Staff reported that children are moved routinely from 8:00 am - 9:00 am in order to maintain ratios. Plan of Correction: School will initiate a system to communicate to parents when we can no longer accommodate children in certain classrooms to maintain ratios. Will limit the amount of movement in the classrooms.
