An unannounced monitoring inspection was conducted on May 22, 2024, as a follow-up to the issuance of a provisional license. The inspector was on site from approximately 12:30 pm until 1:30 pm. There were 45 children present with ten staff supervising. Children were preparing for nap upon the inspector?s arrival. Infants were being feed, rocked, playing in an exerSaucer, and napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. Nine staff records were reviewed as a follow-up and three new staff records were reviewed. Observations and interviews were conducted.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.020 Description: Based on observation and interview, the center did not ensure a copy of the provisional license was prominently displayed by the provider at each public entrance of the subject facility and shall be printed in a clear and legible size and style. In addition, the facility shall be required to prominently display next to the posted provisional license a notice with a description of specific violations of licensing standards to be corrected and the deadline for completion of such corrections is available for inspection at the facility and on the facility's website, if applicable.
Evidence:
1. The Licensing Inspector observed the license that was posted expired on March 6, 2024
2. The Provisional license which was issued on March 7, 2024 was not posted in the center.Plan of Correction: Per the owner: I had the current license on my desk. I just haven't had time to post it. The owner posted the current provisional license while the inspector was on site.
