Based on observations and interview, the center did not maintain all areas and equipment in a clean, safe, and operable condition.
Evidence:

Standard #: 8VAC20-780-280-B Description: Based on observations, the center did not ensure all hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Unlocked sanitizing wipes were located in the Preschool classroom.
2. The kitchen door was propped open. The kitchen contained unlocked cleaning chemicals. The kitchen door opens directly in the classroom. Children were observed in the classroom.

Standard #: 8VAC20-780-330-B Description: Based on observation and measurements, the center did not ensure areas where play equipment is provided, resilient surfacing complied with minimum safety standards described in the American Society for Testing and Materials.
Evidence:
1. A piece of climbing equipment, approximately 42 inches in height, was located on playground. The depth of wood chips underneath the play equipment measured approximately three to four inches in depth at varying locations within the play equipment?s fall zones.
