An unannounced monitoring inspection was conducted on 6/3/24. A review of staff personnel files was conducted by inspectors, Keesha Minor, Jessica Willis, Sarah Zirzow and Mahrukh Aziz at the Campagna office. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on records review, the center employed an individual with non-qualifying fingerprint based criminal record check (CRC).
Evidence:
1. Staff # 1's first day of employment was 8/17/22.
2. The results for Staff # 1's fingerprint based CRC were "not eligible" for employment.Plan of Correction: Employee was terminated effective 6/3/2024
Standard #: 8VAC20-780-240-C Description: Based on records review, staff orientation did not include all required components.
Evidence: Reviewed 8 staff records and found that Staff # 1, Staff # 2, Staff # 3, Staff # 4 and Staff # 5 did not have documentation of all the required facility topics listed in the standards as of 10/13/2021, i.e. safe sleep, procedures to cope with crying babies or distraught children.?Plan of Correction: All staff were trained on SIDS and Head Trauma when the new standard was released by VDOE. Our updated policies are posted on the licensing board at each site. We will also add this to the updated staff orientation form as required, and have the staff resign the updated form.
