An unannounced monitoring inspection was conducted today from approximately 9:20am to 1:15pm. There were 64 children, directly supervised by 15 staff. Staff to child ratio was in compliance. The physical plant, 5 staff records, 5 children records, medications, evacuation drills, emergency supplies were inspected. Children were observed outside, participating in group time and using table toys. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review, for each group of children, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.Evidence: In the JK-B classroom, there were 20 children present, and only 13 children were documented on the written daily attendance. In the JK-A classroom, there were 8 children present, and only 7 children were documented on the written daily attendance.

Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition. Evidence: On the JK-Prep playground there was a portion of the fence toward that back end that was bent backwards. As a result, there were rusted nails protruding from the top of the fence

Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides was not kept in a locked place using a safe locking method that prevents access by children. Evidence: In the Toddler classroom, there was an unlocked cabinet with the following cleaning agents: Rapid Multi Surface Cleaner and Multi Quat Sanitizer. In the Preschool Explorers 2 classroom, there was a bottle of Wizard Air Freshener on the floor in a bin accessible to children.

Description: Based on observation, where playground equipment was provided, resilient surfacing did not comply with minimum safety standards. Evidence: On the JK-Prep playground, there were several holes observed in the resilient surface. On the School Age playground there was not adequate surfacing observed under the fall zones
