An unannounced monitoring inspection was conducted on 6/6/24 from 8:50 a.m. to 10:30 a.m. with the third in charge. There were 37 children in care, ranging in age from two to five-years-old, supervised by six staff. The children were observed in structured play. Five child records and five staff records were reviewed. The Center has staff with current certification in CPR and First Aid, and in Daily Health Observation Training. Five medications and authorization forms were reviewed, and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-80-A Description: Repeat Violation
Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.
Evidence:
1. In the two-year class there was no written record of attendance.
2. In the three-year class there were six children present, the written record reflected there were seven.
3. In the four-year class there were eleven children present, the written record reflected there were ten.Plan of Correction: This took place during morning arrival and all teachers were not settled yet. Have discussed protocol with all staff and they must keep that on going record until the child's teachers have taken their children.
Standard #: 8VAC20-780-270-A Description: Repeat Violation
Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.
Evidence:
1. In the walkway area leading from the building to the playground, peeling and chipping paint was observed on the side of the building.
2. A window screen with sharp edges was in the playground area.Plan of Correction: We do have a handyman that does all repairs and painting during summer and is all scheduled and in process now. Teachers have been told it is important to make supervisor aware of any issues during the school year.
Standard #: 8VAC20-780-330-B Description: Based on observation and measurement, the facility did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.
Evidence: The climbing apparatus located on the playground has less than three inches of pea gravel in the fall zones, six inches is required.Plan of Correction: Children are not using that area as we await landscaping to repair. He had been notified.
Standard #: 8VAC20-780-500-A Description: Based on observation, hand washing procedures after toileting were not always followed.
Evidence: In the two-year class, Licensing Inspector (LI) observed a teacher returning a
child to the group after changing a soiled diaper without washing the child?s hands with soap and running water.Plan of Correction: The teacher in that room has been instructed about the importance of this.
Standard #: 8VAC20-780-500-B Description: Based on observation, the center did not ensure that diapering procedures were always followed.
Evidence: In the two-year class, staff was observed cleaning the diaper changing area after a diaper change. Staff was observed spraying the diaper area with a sanitizing solution, then immediately spraying a soap and water solution to the diaper surface and pulling out a sanitizing wipe to wipe off the mixture. The soap and water solution and the sanitizing solution cannot be combined when disinfecting the diaper surface, they must be used separately. The areas must be cleaned with soap and water, wiped off thoroughly and then sanitizing solution may be applied and allowed to air dry.Plan of Correction: All staff have been retrained on proper procedures as well as new posters hanging at all stations. I am confident that this is not normal as this was first time and inspection had taken place without the director being there and feel staff was nervous.
