Comments:
An unannounced non-mandated inspection was conducted to examine the areas of noncompliance identified in the Intensive Plan of Correction and to follow-up on the provisional license. The inspection was conducted on-site, along with the Licensing Administrator, on June 12, 2024 from 11am until 2:30pm. The director was available during the inspection. There were 158 children in care with 30 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Description: Repeat Violation
Systemic Violation
Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: 1. There was chipped and peeling paint in several rooms and areas of the center. One room contained two holes in the wall and one room had missing drywall around a support column.
2. A playmat in the infant room was ripped in several areas which was causing the vinyl to come off.
3. In several rooms of the center the diaper changing pads and tables that hold the pad contained dirt and debris.
4. Dirt, debris, cobwebs with spider/eggs were found on the floors and stairwell and there were several broken floor tiles throughout the center.
5. The ceiling vents contained built up dirt/dust that expanded to adjacent ceiling tiles.
6. The children's bathrooms downstairs contained trash on the floors, dead bugs in the light fixture, chipped/peeling caulk around one sink and bubbly paint at the baseboard around one toilet.
7. Several rooms in the center contained dirty walls.

Plan of Correction: Center management will complete a center safety checklist weekly to ensure compliance. Teachers will check their classrooms daily and will report anything that is not safe or not operable to management immediately. Center Management will either immediately replace the item or will immediately put in a work order to fix the issue.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure the diapering area shall have a nonabsorbent surface for diapering or changing and failed to ensure disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence: 1. In the Infant B room the diaper changing pad contained a rip in the pad approximately 1 inch long, making it absorbent.
2. In the DPS 1 room there were two diapers located in a trash can that was neither leak-proof or foot-operated. in the children's bathroom.

Plan of Correction: As of 6/12/2024, a new diaper changing pad have been ordered. As of 6/17/2024, the new pad has been installed. As of 6/12/2024 the trash receptacle has been disposed. Currently there are two labeled foot pedal trash receptacles in the classroom. Center management will check both diaper changing pads and trash receptacles weekly in each classroom to ensure they are in proper working order.
