An unannounced monitoring inspection was conducted on 06/17/2024 in response to a self-reported incident received by the licensing office on 06/13/2024 in the areas of staff and supervision and forbidden actions. The inspection included record and policy review, interviews with staff, and a tour of the facility. The inspector arrived at the center at 9:50 am and departed at 11:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/05/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The center shall ensure that the center?s own policies and procedures were followed. The policy stated that behavior management techniques such as enclosing a child in a confined space is unacceptable. Interviews revealed that a staff person placed a 3 year old child in a dark closet with the door closed. Plan of Correction: Director personally spoke to each staff member and retrained them on acceptable forms of discipline and had them sign. If they notice a child is giving an employee a hard time, step up and help out with the child.
Standard #: 8VAC20-780-340-A Description: The center shall ensure that when staff are supervising children, they shall always ensure their care, protection and guidance. Interviews revealed that a staff member picked a 3 year old child up by the arm and then placed the child in a dark closet with the door closed. A staff member could hear the child crying while inside of the closet. Plan of Correction: Director personally spoke to each staff member and retrained them on acceptable forms of discipline and had them sign. If they notice a child is giving an employee a hard time, step up and help out with the child.
Standard #: 8VAC20-780-340-F Description: The center shall ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Interviews revealed that a 3 year old child was not within actual sight and sound supervision of staff during the time that they were placed in the storage closet with the door closed. Plan of Correction: Director personally spoke to each staff member and retrained them on acceptable forms of discipline and had them sign. If they notice a child is giving an employee a hard time, step up and help out with the child.
Standard #: 8VAC20-780-410-2 Description: Enclosing a child in any space that they cannot freely exit and roughly handling a child are forbidden. Interviews revealed that a staff member picked a 3 year old child up off of their cot by their arm and placed them into a dark storage closet with the door closed. Plan of Correction: Director was made aware of the situation at close of business 06/10/2024. After conducting own investigation with the staff member, she was fired upon arrival on 06/11/2024. Director personally spoke to each staff member and retrained them on acceptable forms of discipline. If they notice a child is giving an employee a hard time, step up and help out with the child. Director reviewed forbidden action from the licensing manual with each employee and had them sign.
