Comments:
An unannounced monitoring inspection was conducted on 6/24/2024 between the hours of 09:10am-11:10am. 16 children were observed directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, and policies were inspected. Children were observed playing outside, looking at books, and playing with blocks. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-40-G
Description: Based on record review, the center did not meet the proof of child identity and age requirements as stated in the Code of Virginia.

Evidence:
1. Child #1 began attending the center on 5/20/2024.
2. The record for Child #1 did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: The form used to document the information will be requested and placed in the child's record.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 1 of 4 children's records did not contain all required information.

Evidence:
1. The record for Child #2 did not contain documentation of the child's first date of attendance.

Plan of Correction: The information will be requested and documented.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment inside the center was not maintained in an operable condition.

Evidence:
1. A child was observed sitting in a bumbo seat. The bumbo seat contained safety straps which were not secured.

Plan of Correction: The chair was removed from the play area during the inspection.

Standard #: 8VAC20-780-550-G
Description: Based on record review, documentation of emergency evacuation, shelter in place, and lockdown drills did not include all required information.

Evidence:
The emergency drill records did not include the following information:
1. Identity of the person conducting the drill;
3. The method used for notification of the drill;
6. Any special conditions simulated;
7. The time it took to complete the drill;
8. Problems encountered, if any; and
9. For emergency evacuation drills only, weather conditions

Plan of Correction: The center will record all required information when completing emergency drills.
