A Subsidy Health and Safety Inspection (SHSI) was conducted on 6/27/2024 from 10:40am to 1:30pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on record review and interviews, it was determined the vendor did not ensure that all staff who work directly with children complete within 90 days of the date of employment or 90 days from subsidy vendor approval, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1.The records for staff #3 (date of hire 3/19/2024) and staff #4 (date of hire 1/29/2024) did not include documentation that CPR certification had been completed.

Based on record review and interviews, it was determined that the vendor did not ensure that all staff who work directly with children shall have within 90 days of the date of employment or 90 days from subsidy vendor approval, current certification in first aid appropriate to the ages of children in care.

Standard #: 8VAC20-790-600-F Description: Based on record review and interviews, it was determined the vendor did not ensure that staff who work directly with children shall, in addition to preservice and orientation training, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence:
1.The record for staff #6 did not include documentation that the 2023 health and safety update training had been completed. The latest health and safety training in the record was completed in 2021.
