A complaint was received on 06/28/2024 regarding allegations of supervision. A complaint investigation was initiated on 07/01/2024. Staff interviews were conducted, child and staff files were reviewed and observation of the playground area was made.
The preponderance of evidence did not support a valid finding on the allegation of supervision. During the investigation, one non-compliance was found regarding the center's policies and procedures.

Standard #: 8VAC20-780-40-E Description: Based on a staff written statement and review of an injury report, the licensee did not ensure compliance with the center's own policy on playground safety.
Evidence:
1. The center's playground safety policy requires staff to spread out in zones where there is playground equipment.
2. On 04/25/2024 at approximately 4:25pm, Staff #1 and #2 were positioned in two different zones, when Staff #1 left the playground and entered the center, which left one zone without staff.
3. While Staff #1 was inside, Child #1 fell off a climbing structure which caused a serious injury. The climbing structure was located in the zone that Staff #1 had left from.Plan of Correction: Our staff have been re-trained on
the centers playground policy,
specifically re-instructed to shift
zones as needed to ensure that
the climbing structure is monitored
closely.
Administration will continue to
monitor for compliance and
provide coaching needs and
opportunities as they arise.
