*Probationary status for the facility and what that encompasses.
*Multiple smoke detectors in the facility were "chirping" indicating dead/dying batteries.

Comments:
An unannounced monitoring inspection was initiated on July 1, 2024 and completed on July 17 2024 in response to a self-report received by the licensing office on June 19, 2024 relating to supervision, ratios and forbidden actions. The inspector reviewed 1 child?s record and 3 staff records on site. This inspection included a tour of the facility, documentation review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background searches and the physical plant and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 7, 2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-F
Description: Repeat Violation
Systemic Deficiency
Based on interviews and observation, children under 10 years of age were not always within actual sight and sound supervision of staff..
Evidence:
1. On June 18, 2024 at approximately 9:30 AM, Child 3 (20 months old) and Child 4 (17 months old) exited the Toddler room, to the outdoors into an unfenced area, through an open exterior door. Staff had left the door open while taking out trash. The dumpster is located in the parking lot of the center, approximately 75 feet away from the classroom. Staff stated the children were out of sight and sound supervision for approximately one minute.
2. On July 1, 2024, during the inspection, Child #2 (4 years old) was observed entering the vacant Pre-K (4's) classroom, alone and without supervision, from the vacant Preschool classroom. The child retrieved a jacket and exited the classroom through the back door to the playground area.
3. On July 1, 2024, during the inspection, Adult C (health professional) was observed working alone with Child 1(4 years old) by the inspector upon entering the Pre-K classroom. Adult C is not a staff member and there is no documentation authorizing the individual to pick up the child.

Plan of Correction: Staff members were immediately retrained in the moment on Supervision. Staff then attended and additional training on Supervision and Child Behavior Guidance on 7/10/24. Teachers will continue to receive ongoing monthly training with the next training scheduled for 8/7/24.
Health Care professionals who work with children at the center, based on their family?s request, will have appropriate documentation on file if they will be working alone with a child as part of their individual education plan.

Standard #: 8VAC20-780-350-B-2
Description: Based on interviews, the required staff-to-children ratio was not maintained. The required staff-to-child ratio for children aged 16 months to 24 months is one (1) staff for every five (5) children.
Evidence:
The Toddler room was out of ratio at 1 staff to 7 children at approximately 9:30 AM on June 18, 2024. Staff stated the children were out of ratio for approximately one minute. Staff A left the room to take out the trash leaving Staff B alone in the room with 7 children. The dumpster is in the parking lot approximately 75 feet from the classroom.

Plan of Correction: Staff members will only take trash out through the main entrance of the building. Staff members will use two-way radios to call for a manager to step into support the classroom if a staff member needs to leave the room for any reason.

Standard #: 8VAC20-780-410-1
Description: Based on interviews, children were roughly handled.
Evidence:
1. One June 18, 2024 two children (aged 20 months and 17 months) exited the Toddler room through an open, exterior door.
Staff B stated when she realized the children were not in the room, she ran to grab them and closed the door. Staff demonstrated the hold.
2. Witness #1 stated they observed the staff member, grab the children, yell at them, pull them into the classroom and slam the door.

Plan of Correction: Teachers were retrained on Child Behavior Guidance and strategies for responding to stressful situations. Teachers attended training on 7/10/24 and will continue to receive ongoing monthly training with the next training scheduled for 8/7/24.
