Implementation of a system to check infant bottles.

An inspection was conducted on 7/10/24 to investigate a self reported incident on 6/26/24 related to a milk bottle being fed to the wrong infant in the infant classroom. Two staff were interviewed, 2 staff records and 2 children's records were reviewed along with documentation related to the incident. Based on the preponderance of evidence, violations were cited related to the allegations. Standard #: 8VAC20-780-340-A Description: Based on staff interviews and written documentation, staff supervising children did not ensure their care, protection and guidance.
Evidence:
1. On 6/26/24, between approximately 12:45pm to 1:00pm Child # 1 was given a bottle of breast milk that belonged to Child # 1 in the infant classroom.
2. Staff # 1 stated that when it was time to give Child # 1 to have milk, she grabbed a milk bottle from the fridge that looked identical to Child # 1's bottle, without checking the name on the bottle and properly identifying it.
3. Staff # 1 gave Child # 1 that bottle, who drank more than half the milk contained.
4. When Staff # 2 returned from break she held Child # 1.
5. As Staff # 2 was holding Child # 1 and rotated the bottle she read the name on the bottle and realized it belonged to Child # 2.
6. Staff # 2 stated that Child # 1 drank about 4-5 ounces of the breast milk.Plan of Correction: The infant teachers received additional training on bottle feeding and feeding safety procedures.
