Comments:

An unannounced monitoring inspection was conducted on 07/09/2024. There were 51 children, ages 4 months-5 years, 12 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staff and supervision, physical plant, programs, emergencies, and nutrition. The children were observed in outdoor large motor play, free choice activities, during lunch, and during rest time; infants were observed being fed and allowed to sleep on demand. A total of 5 children?s records, 5 staff records, and 5 medications were reviewed. The inspector arrived for the inspection at 10:15 am and departed the center at 12:30 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	8VAC20-770-60-C-2
Description:	Based on record review, the center failed to obtain the findings from a search of the central registry within 30 days of employment. Evidence: 1. The record for Staff 1 contained a documented hire date of 08/21/2023. There were no findings from a search of the central registry for Staff 1 available for review during the inspection. Staff 1 was observed working during the inspection. 2. The record for Staff 2 contained a documented hire date of 04/08/2024. The search of the central registry for Staff 2 was dated 06/28/2024; the findings were not obtained within 30 days of employment. 3. The record for Staff 3 contained a documented hire date of 04/08/2024. The search of the central registry for Staff 3 was dated 06/28/2024; the findings were not obtained within 30 days of employment. 4. The record for Staff 5 contained a documented hire date of 11/27/2023. The search of the central registry for Staff 5 was dated 01/22/2024; the findings were not obtained within 30 days of employment.
Plan of Correction:	Will now require prior to hire and will place verification of requesting the information in the staff record.
Standard #:	8VAC20-780-500-A
Description:	Based on observation, the center failed to ensure that staff shall wash their hands with soap and running water before feeding or helping children with feeding. Evidence: Staff 6 did not wash their hands before feeding a child a bottle.
Plan of Correction:	Will train staff on handwashing.
Standard #:	8VAC20-780-520-A
Description:	Based on document review, the center failed to ensure that all nonprescription drugs shall be used in accordance with the manufacturer?s recommendations. Evidence: The manufacturer?s instruction for Medication 2 states the following, ?Ages 2-11 years?. The dosage amount for Medication 2, when used for children under 2 years states, ?ask a doctor?. The medication authorization form for Medication 2 did not include dosage instructions from a physician. The medication administration log contained documentation that Medication 2 had been administered to Child 6 (age 13 mo.) on the following dates: 05/01/2024, 05/02/2024, 05/07/2024, 06/03/2024, and 06/12/2024.
Plan of Correction:	The medication will be sent home today. Will require doctors orders before administering again.
Standard #:	8VAC20-780-570-C
Description:	Based on observation and staff interviews, the center failed to ensure that the record for each child on formula shall contain the brand of formula and the child?s feeding schedule. Evidence: Staff 7 stated that there was not a record with the brand of formula and the child?s feeding schedule for each of the 3 children on formula.
Plan of Correction:	Will speak with staff and create a system.
