An unannounced monitoring inspection was initiated and completed on 7/15/2024 in response to a self-report received by the licensing office on 7/12/2024 relating to an injury requiring outside medical attention. The inspector reviewed 4 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Supervision of Children, and background checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/23/2024. A POC submitted after this date will not appear on the public website

Standard #: 8VAC20-770-60-C-2 Description: (Repeat) Providers must obtain the results of a central registry search for a staff member within 30 days of service. Staff #4 provided care to the children beginning 4/17/2024. There were no central registry search results for the staff member during the 7/15/2024 inspection. Plan of Correction: CRS was contacted via email/call. Program manager now has access to provider email.
Standard #: 8VAC20-780-340-D Description: (Repeat) In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. There were 4 staff responsible for a group of 26, 10-12-year-old children, and none of the staff members met the qualifications of a program lead or program director. Plan of Correction: Program manager now requires additional training and understands qualifications requirements.
