An unannounced, on-site monitoring inspection was initiated on 07/19/2024 and completed on 07/19/2024 as a part of the licensure period. The on-site inspection began at 9:22am and ended at 11:42am. The inspector reviewed compliance in the areas listed above. There were 79 children present and 19 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.011-F Description: Based on observations made, the licensee did not ensure that the findings from the most recent inspection were posted in a conspicuous place on the licensed premises.
Evidence:
The inspection report dated 07/01/2024 was not posted.Plan of Correction: Posted the current inspection today.
Standard #: 8VAC20-780-160-C Description: Based on record review, the center did not obtain and submit the results of a follow-up tuberculosis (TB) screening every two years from the date of the first initial screening or testing.
Evidence:
1. Staff #3's TB screening expired on 11/17/2023.
2. Staff #4's TB screening expired on 10/3/2023.Plan of Correction: Will have staff complete and submit a new TB screening.
Standard #: 8VAC20-780-60-A Description: Repeat Violation:
Based on record review and interview, the center did not obtain all the required information for each child's record.
Evidence:
Child #1 is missing addresses for two emergency contacts.Plan of Correction: Will ask the parent to complete the emergency contact information today.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas of the center were not maintained in a clean, safe and operable condition.
Evidence:
The baseboard of the wall in 3 classrooms ( Ponies, Pre-K, and Kittens) was removed exposing peeling paint around the base of the classroom.Plan of Correction: Placed a work order today to get it fixed by 07/22/24.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method.
Evidence:
Clorox and bleach water sanitizing bottles were not kept in a locked place in the Chicks classroom.Plan of Correction: Locked the cabinet during inspection. Will re-train staff during the monthly reminder training.
