An unannounced, on-site monitoring inspection was initiated on July 22, 2024 and completed on July 22, 2024. The on-site inspection began at 1:30pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. There were 56 children present and 8 staff. The inspector reviewed 6 children's records, and 5 staff records on-site. This inspection included document review (i.e. injury logs, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 30, 2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the child day center failed to obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence:
1. The record for Staff #3 (DOH 3/25/24) did not have documentation of an out-of-state sex offender registry results at the time of the inspection.Plan of Correction: The record for staff #3 (DOH 3/25/24) did not have documentation of an out-of-state sex offender registry result at the time of the inspection. Moving forward Out of State background check request will be submitted prior to employees first day of employment.
Violation was corrected during the inspection.
