An unannounced monitoring inspection was initiated on June 24, 2024 and completed on June 24, 2024 in response to a self-report received by the licensing office on June 15, 2024 relating to outside medical treatment. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for the violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 31, 2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-80-B-2 Description: REPEAT
Based on staff interview and documentation, the licensee did not ensure the center inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence: On July 10, 2024, a child #1 was injured and recieved outside medical treatment. The center did not report the injury with in two buisness day. Confirmed by director.Plan of Correction: Director/Owner was out of town at a work related event and reported to inspector and reported injury online as soon as she returned on July 15, 2024.
Director/Owner in the future will contact the inspector phone or email immedialtey when she has knowledge that a child needs Outside Medical treatment. After it is reported to inspector the center will report it online.
