Comments:
An unannounced monitoring inspection was initiated and concluded June 11, 2024 and concluded on June 13, 2024. There were 48 children in care with nine staff. Children and
staff records were reviewed. A walking tour of the center inside and outside was completed. Children were observed in teacher and self directed activities including learning center play activities, outside play, transitions, hand washing, lunch, story, and nap. Additionally, the center reports no medication was being administered at the time of the inspection. Furthermore, emergency evacuation and emergency equipment, annual inspections and insurance were
reviewed as a part of the inspection. Violation regarding the following areas of standards were observed, discussed with the owner administrator and appear on the violation notice: Physical Plant, Staffing and Supervision and Programs.

Description: Based on observation the center failed to ensure that all areas are maintained is safe condition.
Evidence:
1 - The crib sheets in the infant room are not tight fitting and pose an entanglement hazard to sleeping children.
2 - Infants were observed sleeping in cribs with loose fitting sheets.

Description: Based on observation the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Unlocked chemicals were observed throughout the building
- in the infant room 7 spray bottles of disinfectant were on open shelves and unlocked cabinets.
- In the toddler room spray bottles of disinfectant cleaners were stored in unlocked cabinets and on open shelves.
- There were three unlocked chemicals in the two year old room stored above a cabinet.
- There were there unlocked chemicals in the office on shelves within reach of children. The office doors were open and all items accessible in the office during the inspection.

Description: Evidence:
While wondering around the care room crying Child #1, twice began choking on the pacifier in his mouth. The child was crying while holding a pacifier in his mouth. The child's face was wet with tears, there was clear thick mucus coming from his mouth and he was making a loud wailing sound. The staff present were unaware of the situation until it was called to their attention that the child needed assistance.

Description: Based on observation the center failed to ensure that children under 10 years of age are within actual sight and sound supervision of staff.
Evidence:
1 - There were 12 children in care in the preschool room without any staff present. for an unknown length of time. The children present in the room ranged in age from three through 11 years old.
2 - When the staff member returned she stated that she went to answer a phone call.

Description: Based on observation and interview the center failed to ensure that when children are in ongoing mixed age group the staff to child ratio applicable to the youngest child in the group will apply to the entire group.
Evidence:
1 - In the toddler room at approximately 9:30am there were 10 children present on June 11, 2024, ages 12 months to 21 months years old. The number and ages of children present required an additional staff.
2 - Staff present confirmed that there were 10 children and present with the only 2 staff supervising the entire group.
3 - In the preschool room, at approximately 9:30am there were 12 children present ages three to 11 years old with one staff.
4 - The staff present confirmed that she was the only staff assigned to the group.

Description: Based on observation the center failed to ensure that when an infant is placed in his crib, he is placed on his back.
Evidence:
1 - At approximately 10:26am a child in the room was placed in his crib on his stomach.
2 - Staff present stated that she placed the child in the crib on his back.
