An unannounced inspection was initiated on 07/25/2024 and completed on 07/31/2024 in response to a complaint received by the licensing office on 07/16/2024 relating to allegations regarding staffing and supervision, special care provisions and emergencies. This inspection included

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Standard #: 8VAC20-780-550-P Complaint related: No Description: Based on interview and document review, the center failed to main a written record of children?s serious and minor injuries in which entries are made on the day of the occurrence and contain the required information.
Evidence:
1. On 04/11/2024, on Brightwheel (a communication app), the center informed the parent of Child #1 of an injury that occurred to Child #1 while at the center. There was no written record of the injury on file with the required information.
2. On 06/21/2024, on Brightwheel (a communication app), the center informed the parent of Child #1 of an injury that occurred to Child #1 while at the center. There was no written record of the injury on file with the required information.
3. On 07/17/2024, on Brightwheel (a communication app), the center informed the parent of Child #1 of an injury that occurred to Child #1 while at the center. There was no written record of the injury on file with the required information.Plan of Correction: Discuss with each staff member the importance of filling out incident reports, immediately making families aware of injuries and getting them signed before leaving.
