A subsidy inspection was initiated on July 19, 2024 and concluded on July 19, 2024. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-530-C Description: Based on observation and interview, the center did not ensure that the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The written hard copy record of daily attendance for the two year old room only recorded one child?s arrival but there were five children in attendance on 7/19/2024.
2. The pre-K room did not have a completed written hard copy record of daily attendance for 7/19/2024 but had 9 children in attendance.
3. The director, staff #1 confirmed that the written hard copy record of daily attendance were not maintained for the two year old classroom and the pre-k classroom.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-790-600-B Description: Based on record review and interview, the center did not ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

1. The record for staff #3 (hired in 2023) did not contain documentation of a completed 10 hour preservice training.

Standard #: 8VAC20-790-600-F Description: Based on record review and interview, the center did not ensure that staff who work directly with children shall complete annually the department?s health and safety update course.
Evidence:
1. The record for staff #1(hired in 2021), staff #2 (hired in 2023) and staff #3 (hired in 2023) did not contain documentation of a completed 2024 preservice 3 hour training.
