An unannounced on-site monitoring inspection was initiated on 07/31/2024 and completed on 07/31/2024. The on-site inspection began at 10:30 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 24 children present with 4 staff. The inspector reviewed 4 children's records and 3 staff records on-site.
The inspection included:
*Document Reviewed;
*Tour of the facility and outdoor play area;
*Observations.
Information gathered during the inspection determined non-compliance with applicable standards, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Based on record review and interview, the center failed to ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. The most recent TB screening on file for staff 1 was completed on 02/15/2022.
a. Staff 1 was observed caring for children during the inspection.
2. Administrative staff confirmed that an updated TB screening had not been obtained for staff 1.Plan of Correction: Administrative staff stated that staff 1 has an appointment for an updated TB screening on Friday, August 2, 2024.
This was an oversight as it was thought the screening was current.
