An uannounced monitoring inspection was initiated on 5/8/2024 and completed on 8/5//2024 in response to a self-report received by the licensing office on 4/22/2024 relating to Programs. The inspector reviewed 1 staff record. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on t he violation notice Issued to the program.
Please complete a plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today which will be the close of business on 6/6/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Description: Based on observation of video footage, it was determined that the facility did not ensure that staff refrain from using forbidden actions to include roughly handling a child.
Evidence:
On 4/18/2024, between approximately 4:46pm and 4:48 pm, Staff #1 was observed pushing Child #1 (seven years old) out of the way, pulling that child forcibly by the arm, pushing the child in the back of the head and using her body to lean on and push the child away from the area where Staff #1 and two other children were dancing.Plan of Correction: Not available online.
