An unannounced monitoring inspection was initiated on 08/01/2024 and completed on 08/07/2024 in response to a self-report received by the licensing inspector on 07/30/2024 relating to care and protection of children. The inspector reviewed 1 children?s record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/14/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: Based on staff interviews, a staff member failed to follow the center's own policy for reporting suspected abuse and neglect.
Evidence:
Staff # 1 reported an incident of suspected abuse approximately five days after the incident occurred. The center's mandated reporter policy requires staff to report suspected abuse/neglect immediately after being notified, or witnessing an incident of abuse.Plan of Correction: Immediate Action Taken:
Upon being informed of the delay in reporting by Staff #1, I, Hasna Belayachi, the Executive Director, reported the incident to the appropriate authorities within 24 hours of receiving the information.
Staff Training and Policy Reinforcement:
All staff members will undergo mandatory refresher training on the school's mandated reporter policy, emphasizing the importance of immediate reporting. This training will reinforce the legal and ethical responsibilities of all staff members to report any suspected abuse or neglect without delay.
Individual staff meeting will be held to review and discuss the mandated reporter policy to ensure that all staff are fully aware of the procedures and the critical importance of timely reporting.
Monitoring and Accountability:
Acknowledgment of understanding the mandated reporter policy will be required from all staff members following the refresher training.
A protocol will be implemented to document and review any delays in reporting, ensuring that corrective actions are taken immediately.
Ongoing Compliance:
We will conduct regular audits of incident reports to ensure ongoing compliance with the mandated reporter policy.
Periodic checks will be established to verify that all staff members adhere to the immediate reporting requirements.
