An unannounced, on-site monitoring inspection was initiated on 8/2/2024 and completed on 8/2/2024. The on-site inspection began at 9:08 am and ended at 1:08 pm The inspector reviewed compliance in the areas listed above. There were 29 children present and nine staff. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, observations (this would include observations of photos, recordings, etc.), and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/15/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A Description: Based on a review of five staff records, the center failed to ensure that each staff member submitted documentation of a negative tuberculosis screening at the time of employment and prior to coming in contact with the children.
Evidence:
1. The record for staff #1, date of hire 06/3/2024, did not contain documentation of a negative tuberculosis screening prior to employment. Documentation in the record was dated 6/24/2024.
2. The record for staff #2, date of hire 06/10/2024, did not contain documentation of a negative tuberculosis screening prior to employment. Documentation in the record was dated 06/21/2024.

Standard #: 8VAC20-780-270-A Description: Based on observations, areas and equipment of the center, inside and out, were not maintained in a clean, safe and operable condition.
Evidence:
1. Chipping paint was found along the wall, outside the conjoining hall, in the preschool classroom.
2. Chipping paint was found along the wall, near the children?s classroom sink, in the two-year old classroom.
