An unannounced, on-site monitoring inspection was initiated on 07/31/2024 and completed on 08/08/2024. The on-site inspection began at 11:50am and ended at 1:16pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and five staff. The inspector reviewed four children?s records and four staff records electronically on 08/08/2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/19/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the center did not ensure that a copy of the results of a search of the child abuse and neglect registry is obtained from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff 1 indicated on their sworn statement or affirmation that they have resided in a different state within the past five years. There were no results of a search of the child abuse and neglect registry available.
2. Leadership confirmed that the required out-of-state background check was not completed for staff 1.Plan of Correction: I placed the request for a search of the child abuse and neglect registry with Tennessee on 8/13/24. Waiting for results
Standard #: 8VAC20-780-150-B Description: Based on a review of children?s records, the center did not ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
Evidence:
1. The record for child #2 contained a physical and immunization record that was not signed by the
physician, his designee, or an official of a local health department.Plan of Correction: Staff will review all files and confirm all physical and immunization records have the appropriate signatures.
Standard #: 8VAC20-780-550-P Description: Based on review of injury reports, the center?s written records of children's injuries did not include all required information.
Evidence:
Five minor injury reports were reviewed and were missing following information:
1. One report, dated 07/24/2024, was missing the future action to prevent injury and time of injury
2. One report, dated 06/25/2024, was missing how parent was notified and two signatures.Plan of Correction: The Club Director and the administrative associate will review all injury reports for completion.
