An unannounced, on-site monitoring inspection was initiated on 8/8/2024 and completed on 8/8/2024. The on-site inspection began at 9:10am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 54 children present and 11 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/20/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Based on review of children?s records, the center did not ensure each child record contained the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. The record for child #1 contained only one emergency contact listed in his file and on his emergency card that was not a parent.
2. Staff acknowledged only one emergency was contact listed and obtained an additional emergency contact while inspector was on-site.Plan of Correction: The correction was resolved on-site to ensure compliance. The parent provided management with an additional emergency contact. Management has implemented a system to double-check all child files to ensure compliance prior to a child's first day of attendance.
Standard #: 8VAC20-780-290-A-3 Description: Based on observations and interview, the center did not ensure electrical outlets shall have protective covers.
Evidence:
1. During the inspection, an electrical outlet in the two-year old?s classroom was observed with the protective cover in the open position with the outlet exposed posing a safety hazard to children.
2. Staff acknowledged the teachers in the classroom are responsible for ensuring the sliding outlet covers are closed when they are not in use.Plan of Correction: Management directed staff to install an electrical outlet cover and/or tape until maintenance arrives to complete repairs. Prior to the visit, management submitted a worker order on 8/5/2024.
