An unannounced, on-site renewal inspection was initiated on July 23, 2024, and completed on July 23, 2024, as a part of the provisional licensure period. The on-site inspection began at 2:00 pm and ended at 4:30 pm. The inspector reviewed compliance in the areas listed above. There were 51 children present and 11 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 17, 2024. A POC submitted after this date will not appear on the public website

Based on review of seven staff records and interview, the center did not obtain a completed sworn statement prior to employment for one staff member.
Evidence:
1. The record of staff #1, employed 06/03/2024, did not contain documentation of a completed sworn statement exceeding the first day of employment.

Based on review of seven staff records, the center did not obtain the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening for one staff member.

Based on review of seven staff records, the center did not obtain all required documentation for one staff record.
Evidence:
The record of staff #1, employed 06/03/2024, did not contain the name, address, and telephone number of a person to be notified in an emergency; two or more references as to the character and reputation as well as competency; verification of age; orientation training in first aid and cardiopulmonary resuscitation; and documentation of orientation training.Plan of Correction: Not available online. Standard #: 8VAC20-780-350-M Description: Based on observation and interview, the center did not ensure one staff member shall not supervise more than one room or area during rest time.
Evidence:
On 07/23/2024, staff #8 was observed supervising nine napping children, in the three-year-old area. The room is divided into two areas, the three-year-old class and the four-year-old class, by cabinets, approximately six feet in height along with a curtain that creates a wall. Five napping children were observed unsupervised in the four-year-old area. Staff #8 stated she was watching both areas. Standard #: 8VAC20-780-510-L Description: Based on observation and interview, the center did not ensure medications, except for those prescriptions designated otherwise by written physician?s order, were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
The director extracted four children?s medications from the unlocked desk drawer in the office. Three medications were contained in approved child safety locked bags. The medication for child #6 was contained in a zip-lock bag. Two children were observed in the office playing where the unlocked medication was being kept.
