An unannounced monitoring inspection was initiated on 8/1/24 in response to a self-report received by the licensing office on 7/25/24 relating to forbidden actions. This inspection included document review, tour of the classroom, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to behavioral guidance and inspection posting and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/19/24. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.011-F Description: (Repeat Violation) Based on observation and staff interview, the center did not ensure they post the findings of the most recent licensing inspection of the facility.
Evidence: The most recent licensing inspection of the facility from the inspection completed on 7/19/24 was not posted.Plan of Correction: The inspection report summary that was posted did not contain the entire inspection report. The complete Inspection report was posted during the licensing visit on 08/01/2024
Standard #: 8VAC20-780-400-B Description: Based on staff interviews, the Center failed to use behavioral guidance that was constructive in nature, age and stage appropriate, or intended to redirect a child to appropriate behavior and resolve conflicts.
Evidence:
Interviews revealed that Staff #1 yells daily at the children in the Preschool 2A classroom (ages 2-3 years old) when they become frustrated with the children. The staff member has been seen and heard yelling the phrases ?sit down?, ?leave it alone?, and ?move? and sometimes will yell while getting close up to the children?s faces.Plan of Correction: The staff member has received additional training on developmentally appropriate child guidance.
