Standard #: 8VAC20-780-280-B Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the Stem classroom there were 2 unlocked upper cabinets, in one cabinet was a box of laundry detergent and in the other cabinet was a bottle of disinfectant.Plan of Correction: Ensure cabinets are locked at all times.

Based on staff interviews, staff did not ensure that children under 10 years of age always be within actual sight and sound supervision of staff.
Evidence:
1. Interviews with staff determined that on 7/25/2024 at approximately 4:30pm, Child A (4 Years Old) was left alone and unsupervised in the Library Room for less than 1 minute.
2. Child A then walked out of the library room and was discovered by another staff in the hallway.
3. Child A?s classroom had left and was already in the Music Classroom when the child was discovered in the hallway.
4. The Music classroom was not within sight and sound and was around the corner approximately 47.3 feet away from the Library Room.Plan of Correction: Retrained all team members on CSR rules and safety regulations.
