Comments:
An unannounced, on-site subsidy health and safety inspection (SHSI) was initiated on 07/29/2024 and completed on 08/05/2024, as a part of the licensure period. On 07/29/24, the on-site inspection began at 11:10am and ended at 3:45pm. The inspector reviewed compliance in the areas listed above. There were 133 children present and 24 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. An additional 23 staff records were reviewed on-site on 08/05/2024. This inspection included document review, tour of the facility, interviews, and observations.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/03/2024.

Standard #: 8VAC20-790-600-B
Description: Based on a review of 33 staff records, the vendor did not ensure one staff member who works directly with children shall complete the Virginia Preservice Training for Child Care Staff within 90 days of employment.

Evidence: Staff #27, date of employment 05/30/19, completed the Virginia Preservice Training for Child Care Staff on 07/17/24.

For staff employed prior to October 13, 2021, and approved as subsidy vendors, the Virginia Preservice Training for Child Care Staff was required to be completed within the first 90 days of employment.

Plan of Correction: Per the Center: "The staff member has a copy of Preservice Training in their file. Staff members will complete this form within 90 days of employment.

School management conducted an internal review that resulted in the file being compliant with the completed format the time of the licensing visit.

The management responsible for the lack of timely completion has separated from the company."

Description: Based on a review of 33 staff records and interview, the vendor did not ensure three staff members had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment.

Evidence: 1) The CPR certification in the record of Staff #15, date of employment 03/08/24, was completed on 07/20/24.

2) The CPR certification in the record of Staff #28, date of employment 01/29/24, was completed on 07/20/24.

3) The CPR certification in the record of Staff #32, date of employment 01/15/24, was completed on 07/20/24.

4) During interview, management confirmed the staff members did not have current certification in CPR within 90 days of the date of employment.

Description: Based on a review of 33 staff records and interviews, the vendor did not ensure seven staff members who work directly with children completed the department's health and safety update course annually.

Evidence: 1. The record of Staff #4, date of employment 09/21/21, did not contain documentation that the staff completed the department's health and safety update course from 2021 - 2023. The most recent certificate was dated 07/11/24.

2. The record of Staff #8, date of employment 10/03/22, did not contain documentation that the staff completed the department's health and safety update course in 2023. The most recent certificate was dated 07/18/24.

3. The record of Staff #10, date of employment 10/05/22, did not contain documentation that the staff completed the department's health and safety update course from 2020 - 2023. The most recent certificate was dated 04/20/24.

4. The record of Staff #11, date of employment 04/23/08, did not contain documentation that the staff completed the department's health and safety update course from 2021 - 2023. The most recent certificate was dated 07/29/24.

5. The record of Staff #19, date of employment 08/16/21, did not contain documentation that the staff completed the department's health and safety update course from 2022 - 2023. The most recent certificate was dated 07/12/24.

6. The record of Staff #20, date of employment 10/03/22, did not contain documentation that the staff completed the department's health and safety update course in 2023. The most recent certificate was dated 07/16/24.

7. The record of Staff #25, date of employment 04/23/19, did not contain documentation that the staff completed the department's health and safety update course from 2022 - 2023. The most recent certificate was dated 07/21/24.

8. During interviews, management reported the center did not have additional documentation
to verify the required trainings were completed annually for each staff member.

Plan of Correction: Per the Center: "The staff members have a copy of Health and Training in their file for 2024. Staff members will completed this form annually.

School management conducted an internal review that resulted in the files being compliant with 2024 annual training documentation at the time of the licensing visit.

The management responsible for the lack of timely completion has separated from the company."
