Technical assistance was provided in the following areas: 780-60 A Children's records.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site focused inspection was initiated and completed on August 22, 2024, as a part of probation. The on-site inspection began at 10:00am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 65 children present and 16 staff. The inspector reviewed five children?s records and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 4, 2024. A POC submitted after this date will not appear on the public website.

Based on record review the center did not ensure that children?s records contain all the required information.
Evidence:
1. The record for Child #1 and Child #4 was missing the work phone number for one parent.
2.The record for Child #2 was missing names of persons authorized to pick up the child.
3.The record for Child #3 was missing the work phone numbers for both parents.Plan of Correction: Missing information & file updates completed & on file for child #1, child #2, child #3 & child #4 on 08/23/2024. Moving forward a paperwork appointment will be conducted with each family to ensure all files are in compliance prior to child?s first day of enrollment.
