An unannounced, on-site monitoring inspection was initiated on August 29, 2024 and completed on August 29, 2024, as a part of current licensure period. The on-site inspection began at 8:05am and ended at 9:30am. The inspector reviewed compliance in the areas listed above. There were 32 children present with 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site.
This inspection included:
? document review of policies and procedures, emergency drill logs, medication
administration logs, authorization forms,
? tour of the facility,
? interviews,
? observations of the children playing on playground, eating breakfast, preparing to leave for the school day.
Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on record review, the center did not ensure that each child's records contained complete documentation of all required information.
Evidence:
1. The record for child #2 was missing the address for both of two required emergency contacts. The name, address, and phone number of a person to contact in an emergency if the parents can not be reached is required.Plan of Correction: The facility will obtain the complete information needed for each child's file going forward.
