An unannounced, on-site monitoring inspection was initiated on 08/28/2024 and completed on 08/28/2024. The on-site inspection began at 9:01am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 11 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/12/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the center did not ensure that a copy of the results of a search of the child abuse and neglect registry is obtained from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #3 (DOH 12/12/2023) indicated on their sworn statement or affirmation that they have resided out of state within the past five years. The request for central registry was sent on 11/27/2023.
3. There were no results of a completed search of the child abuse and neglect registry in the staff record.

Standard #: 8VAC20-780-130-E Description: Based on a review of children?s records, the center did not ensure that each child?s record
contained documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #4, 2 years of age, had immunization records dated 06/12/2023. Child?s first day of attendance was 11/20/2023. An updated immunization was required in May of 2024.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-70 Description: Based on record review, the center did not ensure staff records contained documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.
Evidence:
1. The record of staff #2 (Date of Hire 08/30/2023) did not contain documentation of 24 hours of program leader training.
2. Administration reported that staff #2 is a program leader and they have not documented the required training in the staff's record.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-500-A Description: Based on observation, staff did not ensure children's hands were washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence:
1. In classroom 1, children were observed eating morning snack. When children at the table finished snack, they were instructed by staff to sit at the carpet.
